Are there any limitations with pre-existing conditions?
A Pre-existing Condition is not covered
unless the date of diagnosis for such
condition is at least 12 months after the

covered person received medical advice or
treatment within the 12 months preceding
the certificate effective date.
How can the benefit payment be used?

Do I have to complete a medical exam?
You do not have to complete a medical exam, however, a pre-existing condition is not covered
unless the date of diagnosis for such

covered person received medical advice or
treatment within the 12 months preceding
the certificate effective date.
Chubb Critical Illness is guaranteed issue up to the policy maximum of $30,000 for members and 50% of member face amount for spouses and children.
